We want your first day at Winthrop Physical Therapy go as smoothly as possible. The following is a list of items we will most likely need from you before you get started:

New Patient Paperwork - Please download all required forms from the Patient Paperwork column on the right. Print and complete prior to your visit. If unable, please arrive 15 minutes early to complete on the day of your appointment.

Insurance Information and Cash Pay Option - As a convenience to you, we verify every patient's insurance coverage. Insurance verification is not a guarantee of benefits, and we suggest each patient personally verify insurance details with their respective provider to ensure patient familiarity with physical therapy coverage. If you would like to bypass insurance and participate in a cash pay option, please call and speak to our front desk staff for details.

Referrals and Doctor Prescriptions - The State of Washington allows direct access for patients to physical therapy. This means no referral from your doctor is required for us to provide care. However, some insurance companies do still require a referral. If you are unsure of your health insurance requirements, please call our front desk to help answer any questions you may have.

Appropriate Clothing - In order for our therapists to conduct a thorough examination, they must first have access to the area of injury. We recommend wearing loose clothing, sports bras, tank tops and/or shorts to your first appointment. This will allow us to better assess your condition and how different parts of your body relate to your injury.
